Kinship Child & Youth Application

Dear Parent or Guardian,

Please fill out the application below as completely as possible. Information provided will be kept safe and confidential in our secure database. Only basic details that will help us determine the suitability of your child may be shared with approved Kinship staff and volunteers. During the matching process, basic details about your child’s personality, needs, and preferences will be shared with a potential mentor to assist them in determining if they would be a good fit. However, the child’s full name, and his/her family members, will be kept confidential until all parties agree to a match. This process helps ensure that the wishes of all parties are respected, yet maintains privacy should a match be declined by either party.
