Photo/Media Release Form

I understand that Kinship of the Park Rapids Area often takes photographs at the time my child is enrolled in the program, at the time of his/her match, and at Kinship events and parties. These photographs may be used at a later time for publicity/promotion purposes. I give permission for Kinship of the Park Rapids Area to use my child’s photo for publicity/promotion purposes. This may also include publication in a local newspaper, magazine, the Kinship website, social media, or poster/billboard. If names are used, my child will be identified by first name only, unless specific permission to use first and last names is granted.
